A two-step process:
1. Fill out a form
2. Submit payment

Membership in Impact Fairfield County is conditioned upon receipt of a $1,000 non-refundable donation and this signed form. Contributions are due by December 31st if you would like to participate in the early round of reviewing the non-profit applications.

Once completing the fields below, submit the form by clicking on the white button below. Once submitted, a completion notice will appear. If you do not see the notice, the form has not been submitted.
Then proceed to Step 2 for payment.
